Cause Released on Fatal Ohio Fair Incident

Excessive corrosion on a support beam caused a "catastrophic failure" of a thrill ride at the Ohio State Fair that killed an 18-year-old man and injured seven others, the ride's Dutch manufacturer said in a statement posted Sunday on Facebook.

The ride's manufacturer, KMG, announced investigation results Sunday, almost two weeks after the accident.

It said the corrosion "dangerously reduced" the thickness on the wall of the beam holding a passenger gondola on the swinging and spinning ride. "On the Afterburner in OH we did the maintenance until a few years ago", Kroon said at the time.

All rides at the Ohio State Fair in Columbus were shut down after 18-year-old Tyler Jarrell was killed after the Fire Ball ride he was on fell apart in mid-air on July 26. Franklin County Coroner Anahi Ortiz said that Jarrell died of blunt force trauma injuries to the head, trunk and lower extremities.

Ohio Department of Agriculture records showed passing marks on inspections for Fire Ball on about three dozen items, including possible cracks, brakes, proper assembly and installation. FOX8 Cleveland reports that four others remain hospitalized, including Jerrell's girlfriend, Keziah Lewis, 18.

The rides at the fair were initially closed after the incident but were later reopened.

The ride cleared inspection just hours before it malfunctioned. The fair's last day was Sunday.

Jarrell's family has hired a civil attorney to pursue a wrongful death lawsuit, according to the Associated Press.

Ohio Gov. John Kasich ordered all the fair's rides shut down for a new round of safety inspections in the immediate aftermath.
